Health Care Services Master Plan Consistency Determination
Pursuant to Planning Code Section 342, certain Medical Use projects must complete and submit a Health Care Services Master Plan Consistency Determination Application as part of any entitlement or building permit application.
Please reference the Health Care Services Master Plan Recommendations and Guidelines when completing this application.
