Application Intake Appointment Request
To schedule an intake appointment, please send an email request with this form attached to: CPC.Intake@sfgov.org.
You will receive a confirmation email within three business days offering the next date and time available. The message will also include any further instructions related to your submittal.
The PDF version of this form is fillable. To save any text entered in the form, first download it to your computer or network drive (right-click or ctrl-click) then complete and save the file. If you fill out the form using only the browser (Chrome, Safari, Internet Explorer, etc) the text is not retained.
